HGE Healthcare: a telemedicine platform that catches a COPD flare before it becomes an ER visit

Resourcifi built HGE Healthcare, a remote COPD management platform that pairs a calm daily check-in for patients with a real-time risk dashboard for clinicians, so a rising symptom triggers an alert and a call instead of an admission.

The challenge

What HGE Healthcare needed to solve.

People living with COPD often slide toward a flare for days before anyone notices, and by then the only option left is the emergency room. HGE Healthcare needed daily symptom reporting that patients would actually keep up with, paired with a nurse and clinician view that surfaces the riskiest patients first, so a care team can step in early. The whole system had to feel clinical, calm, and trustworthy on both iOS and Android.

Goals
  • Make a daily symptom check-in quick enough to keep a habit
  • Flag rising risk automatically and route it to the care team
  • Give clinicians a real-time list sorted by who needs review
  • Connect patients to telehealth visits without leaving the app
  • Engineer the platform to safeguard sensitive health data
The solution

Resourcifi designed and built HGE Healthcare as native iOS and Android apps on a clinical data platform with real-time alerting. Patients see a single wellness score, a ninety second check-in, and their vitals; clinicians see a triage list where high risk, monitor, and stable patients are colour coded and a week of daily status reads at a glance. When a check-in trends the wrong way, an automated alert moves the patient up the clinician's queue and into a telehealth visit.

What we built

The HGE Healthcare build, screen by screen.

01 · Clinician triage and home

The riskiest patients, surfaced first

The clinician home opens on a count of high risk, monitor, and stable patients, then a review list where each patient carries a risk badge and a week of colour coded daily status.

  • High risk, monitor, and stable counts
  • Priority, all, and flagged tabs
  • A week of daily status squares
  • Latest vitals and a review action

02 · Patient home and wellness

One clear score for the day

The patient home leads with a green wellness score and a plain reading of how breathing looks today, followed by the daily check-in prompt and the latest heart rate and oxygen vitals.

  • Today's wellness score with a ring
  • A plain-language status line
  • Daily check-in prompt with Start
  • Heart rate and SpO2 vitals

03 · Daily check-in

A check-in that takes about ninety seconds

The daily check-in asks how breathing feels on a simple slider, then a few symptom chips, so a patient can report in under two minutes and the result feeds the risk engine.

  • Breathlessness on a guided slider
  • Tap to add today's symptoms
  • Inhaler and medication prompts
  • Submitted in about ninety seconds

04 · Vitals and trends

Vitals that read over time

The trends view holds oxygen and heart rate with a ring and a chart, so a patient and their care team can see whether the last week is steady, improving, or sliding.

  • SpO2 ring with the current reading
  • Heart rate over the week
  • Steady, improving, or warning state
  • Shared with the care team

05 · Telehealth visit

A visit with the care team in app

When a flag goes up, the patient sees their next telehealth visit, the care team on the call, and a single button to join the video, all without leaving the app.

  • Upcoming visit with date and time
  • Care team and assigned nurse
  • One tap to join the video
  • Notes shared back to the record

06 · Care plan and profile

Care plan, education, and account

The profile holds the patient's COPD care plan, education tied to their condition, connected vitals devices, and privacy controls on a platform engineered to safeguard health data.

  • COPD care plan and goals
  • Education for the condition
  • Connected vitals devices
  • Privacy and data controls
